Contact our office at 260-338-1241. We will take basic information over the phone and provide you with our Intake/Registration form via postal mail or email per your request.
You may be asked to assist us with obtaining insurance approval.
Once your Registration form is received and insurance approval is obtained, we will schedule your initial appointment.
